Biomechanics of coupled motion in the cervical spine during simulated whiplash in patients with pre-existing cervical or lumbar spinal fusion

چکیده
OBJECTIVES Loss of motion following spine segment fusion results in increased strain in the adjacent motion segments. However, to date, studies on the biomechanics of the cervical spine have not assessed the role of coupled motions in the lumbar spine. Accordingly, we investigated the biomechanics of the cervical spine following cervical fusion and lumbar fusion during simulated whiplash using a whole-human finite element (FE) model to simulate coupled motions of the spine. METHODS A previously validated FE model of the human body in the driver-occupant position was used to investigate cervical hyperextension injury. The cervical spine was subjected to simulated whiplash exposure in accordance with Euro NCAP (the European New Car Assessment Programme) testing using the whole human FE model. The coupled motions between the cervical spine and lumbar spine were assessed by evaluating the biomechanical effects of simulated cervical fusion and lumbar fusion. RESULTS Peak anterior longitudinal ligament (ALL) strain ranged from 0.106 to 0.382 in a normal spine, and from 0.116 to 0.399 in a fused cervical spine. Strain increased from cranial to caudal levels. The mean strain increase in the motion segment immediately adjacent to the site of fusion from C2-C3 through C5-C6 was 26.1% and 50.8% following single- and two-level cervical fusion, respectively (p = 0.03, unpaired two-way t-test). Peak cervical strains following various lumbar-fusion procedures were 1.0% less than those seen in a healthy spine (p = 0.61, two-way ANOVA). CONCLUSION Cervical arthrodesis increases peak ALL strain in the adjacent motion segments. C3-4 experiences greater changes in strain than C6-7. Lumbar fusion did not have a significant effect on cervical spine strain.Cite this article: H. Huang, R. W. Nightingale, A. B. C. Dang. Cervical spine curvature during simulated whiplash.
OBJECTIVE To develop a new method to describe cervical spine curvature and evaluate the potential for injury in the upper and lower cervical spine during simulated whiplash. DESIGN A method was developed to integrate the upper and lower cervical spine rotations and describe the spine curvature. BACKGROUND In vivo and in vitro whiplash simulations have documented the development of an S-shap...
متن کاملWhiplash: a review of a commonly misunderstood injury.
Whiplash injury is a relatively common occurrence, but its mechanism and optimal treatment remain poorly understood. It is estimated that the incidence of whiplash injury is approximately 4 per 1,000 persons. The most common radiographic findings include either preexisting degenerative changes or a slight flattening of the normal lordotic curvature of the cervical spine. Computed tomography and...
متن کاملKinematic Magnetic Resonance Imaging Assessment of the Degenerative Cervical Spine: Changes after Anterior Decompression and Cage Fusion
Study Design A prospective cohort study. Objective Decompression and fusion of cervical vertebrae is a combined procedure that has a high success rate in relieving radicular symptoms and stabilizing or improving cervical myelopathy. However, fusion may lead to increased motion of the adjacent vertebrae and cervical deformity. Both have been postulated to lead to adjacent segment pathology (ASP)...

STUDY DESIGN A systematic review. OBJECTIVE To determine whether different indications or reasons for spinal fusion are associated with different risks of subsequent adjacent segment pathology (ASP) in the lumbar and cervical spine. SUMMARY OF BACKGROUND DATA Pre-existing degeneration at levels adjacent to an arthrodesis may play a role in the development of symptomatic adjacent segment pat...
متن کاملExtremely Large Amount of Fused Segments in a Rare Case of Klippel-Feil Syndrome.
Klippel-Feil Syndrome (KFS) is a complex, congenital condition, originally described by Maurice Klippel and Andre Feil in 1912.1 It is characterized by congenital fusion of two or more cervical vertebrae. Because of the abnormal fusion process and altered spinal biomechanics with undue motion at the non-fused cervical segment, adjacent to fused segments, such individuals are predisposed to abno...
